Nach vorgängiger Kostengutsprache durch den Krankenversicherer nach vorheriger Konsultation des Vertrauensarztes.
Eine Kostengutsprache hat den entsprechenden Indikationscode (21276.01) zu enthalten.
POLIVY ist in Kombination mit Bendamustin und Rituximab zur Behandlung erwachsener Patienten mit rezidiviertem oder refraktärem diffusem grosszelligem B-Zell-Lymphom (Diffuse Large B-Cell Lymphoma, DLBCL) indiziert, die nicht für eine hämatopoetische Stammzelltransplantation infrage kommen.
Die Behandlung mit POLIVY wird für maximal 6 Therapiezyklen vergütet oder kürzer, sofern eine Krankheitsprogression oder inakzeptable Toxizität vor Abschluss der 6 Therapiezyklen auftritt.
Für POLIVY und Rituximab bestehen Preismodelle. Die Zulassungsinhaberin erstattet dem Krankenversicherer, bei dem die versicherte Person zum Zeitpunkt des Bezugs versichert war, auf dessen erste Aufforderung hin, für jede bezogene Packung POLIVY einen festgelegten Anteil des Fabrikabgabepreises zurück. Sie gibt dem Krankenversicherer die Höhe der Rückerstattung bekannt.
Die Mehrwertsteuer kann nicht zusätzlich zu diesem Anteil des Fabrikabgabepreises zurückgefordert werden. Die Aufforderung zur Rückerstattung soll zeitnah zur Verabreichung erfolgen. Ohne Rückerstattungen sind POLIVY und Rituximab in dieser Kombination nicht wirtschaftlich.
Eine Vergütung von POLIVY kann nur mit einem Rituximab Kombinationspartner erfolgen, dessen Limitation die Kombinationstherapie (POLIVY und Rituximab und Bendamustin) zur Therapie des DLBCL explizit aufführt. Hingegen erfolgt eine Vergütung der Kombination, wenn bei Präparaten mit dem Wirkstoff Bendamustin die Kombination mit POLIVY und Rituximab in deren Limitierung nicht explizit aufgeführt ist.
