Limitation
This preparation is reimbursed only under the conditions below. The text is the binding FOPH wording; outside those conditions basic insurance does not pay.

Radiojod-refraktäres, lokal fortgeschrittenes oder metastasierendes, progredientes, differenziertes Schilddrüsenkarzinom.
Es werden nur die Dosierungen gemäss Fachinformation vergütet (24 mg, 20 mg, 14 mg, 10 mg oder tiefere Dosierungen).
Folgender Indikationscode ist an den Krankenversicherer zu übermitteln: 20408.01
Als Monotherapie zur Erstlinientherapie des fortgeschrittenen, nicht-reserzierbaren Leberzellkarzinoms bei Patienten, für welche alle folgenden Kriterien zutreffen:
- weniger als 50% der Leber sind vom Karzinom befallen
- es liegt keine Tumorinvasion in den Gallengang oder den Hauptportalvenenarm der Pfortader vor
- ECOG Status 0-1
- Child-Pugh A
- BCLC-Stadium B, wenn eine TACE (transarterielle Chemoembolisation) nicht infrage kommt oder BCLC-Stadium C
- Keine gleichzeitige Verabreichung von HAIC (hepato-arterielle Infusion einer Chemotherapie).
Es werden nur die Dosierungen von einmal täglich 12 mg, 8 mg oder 4 mg gemäss Fachinformation vergütet.
Folgender Indikationscode ist an den Krankenversicherer zu übermitteln: 20408.02
LENVIMA in Kombination mit Pembrolizumab ist vergütet zur Behandlung des fortgeschritten Endometriumkarzinoms, das nicht MSI-H oder dMMR ist, bei erwachsenen Patientinnen, bei denen die Erkrankung nach einer vorherigen platinbasierten Therapie progredient ist und für die ein kurativer chirurgischer Eingriff oder Bestrahlung nicht in Frage kommt.
Eine Vergütung von LENVIMA kann nur mit dem Kombinationspartner Pembrolizumab erfolgen, dessen Limitation die Kombinationstherapie (LENVIMA + Pembrolizumab) in der Indikation 2L Endometriumkarzinom explizit aufführt.
Die Eisai Pharma AG vergütet dem Krankenversicherer, bei dem die versicherte Person zum Zeitpunkt des Bezugs versichert war, auf dessen erste Aufforderung hin für jede bezogene Packung LENVIMA einen festgelegten Anteil des Fabrikabgabepreises zurück. Die Eisai Pharma AG gibt dem Krankenversicherer die Höhe der Rückvergütung bekannt. Die Mehrwertsteuer kann nicht zusätzlich zu diesem Anteil des Fabrikabgabepreises zurückgefordert werden.

Yes, but only under conditions. Lenvima is on the Specialities List with a limitation: basic insurance reimburses it only when the conditions in the limitation text are met, in some cases only after the insurer has granted cost approval.
