This preparation is reimbursed only under the conditions below. The text is the binding FOPH wording; outside those conditions basic insurance does not pay.

• einem unzureichenden Ansprechen einer ESA-Therapie (sog. ESA Hyporesponder oder bei ESA-Resistenz). Für die Bestätigung eines Hyporesponders muss entweder eine Dokumentation vorliegen, dass er/sie trotz hoher ESA-Dosen nicht die gewünschte Hämoglobin-Konzentration erreicht bzw. diese nicht aufrechterhalten kann oder es muss eine Dokumentation vorliegen, dass er/sie zunehmend höhere ESA-Dosierungen benötigt, um den Ziel-Hb-Wert zu erreichen.
ODER
• einer dokumentierten Unverträglichkeit einer ESA-Therapie, die die Weiterbehandlung mit einem ESA unmöglich macht.
Die Behandlung mit VAFSEO bedarf einer Kostengutsprache durch den Krankenversicherer nach vorgängiger Konsultation des Vertrauensarztes.
Die Erstverordnung bis zum Erreichen eines stabilen Ziel-Hb-Werts muss von einer Fachärztin/einem Facharzt für Nephrologie unter engmaschiger Kontrolle durchgeführt werden.
Die Therapie mit VAFSEO ist abzubrechen, wenn innerhalb von 24 Wochen nach Beginn kein klinisch bedeutsamer Anstieg der Hb-Werte erzielt wird.
