Morbus Crohn
Die Verschreibung kann nur durch Fachärzte der Gastroenterologie oder gastroenterologische Universitätskliniken/Polikliniken erfolgen.
Erhaltungstherapie zur Behandlung erwachsener Patienten mit mittelschwerem bis schwerem aktivem Morbus Crohn, bei denen konventionelle Therapien oder die Behandlung mit einem Biologikum ungenügend angesprochen haben, nicht mehr ansprechen, kontraindiziert sind oder nicht vertragen wurden.
Es wird maximal eine Dosis von 300 mg (Fertigpen, 100 mg/ml, 1 Stk + Fertigpen, 200 mg/2 ml, 1 Stk.) pro Verabreichung alle 4 Wochen vergütet.
Eine Weiterbehandlung mit OMVOH nach einem Jahr bedarf der Kostengutsprache durch den Krankenversicherer nach vorgängiger Konsultation des Vertrauensarztes.
Es wird ausschliesslich die Packung 100 mg /ml + 200mg / ml je ein Fertigpen für die Erhaltungsphase in der Indikation Morbus Crohn vergütet.
Die Firma Eli Lilly (Suisse) SA erstattet dem Krankenversicherer, bei dem die versicherte Person zum Zeitpunkt des Bezugs versichert war, auf dessen erste Aufforderung hin für jede zur Behandlung in der Indikation Morbus Crohn bezogenen Packung OMVOH einen Anteil von Fr. 108.80 pro Packung zurück. Die Mehrwertsteuer kann nicht zusätzlich zu diesem Anteil des Fabrikabgabepreises zurückgefordert werden. Die Aufforderung zur Rückvergütung soll ab dem Zeitpunkt der Verabreichung erfolgen.
Der Indikationscode dieser Limitierung ist an den Krankenversicherer zu übermitteln.
