Limitation
This preparation is reimbursed only under the conditions below. The text is the binding FOPH wording; outside those conditions basic insurance does not pay.

1a. Zur Prophylaxe von invasiven Mykosen (insbesondere Aspergillose) bei neutropenischen Patienten mit akuter myeloischer Leukämie oder myelodysplastischem Syndrom, die eine zytotoxische Chemotherapie erhielten.
1b. Zur Prophylaxe von invasiven Mykosen (insbesondere Aspergillose) bei Patienten mit Graft versus Host Disease (Transplantat-gegen-Empfänger-Reaktion).
2. Fusariose bei Patienten, deren Erkrankung therapieresistent ist gegenüber Amphotericin B bzw. bei Unverträglichkeit von Amphotericin B (2nd line). 3. Chromoblastomykose und Myzetom bei Patienten, deren Erkrankung therapieresistent ist gegenüber Itraconazol bzw. bei Unverträglichkeit von Itraconazol (2nd line).
4. Kokzidioidomykose bei Patienten, deren Erkrankung therapieresistent ist gegenüber Amphotericin B, Itraconazol oder Fluconazol bzw. bei Unverträglichkeit dieser Arzneimittel (2nd line).
