Die Behandlung mit CABLIVI hat bis zu einem ADAMTS13 Aktivitäts-Schwellenwert von mindestens 20% mit einem bestätigten Aufwärtstrend (bestimmt anhand von 2 Messungen aus dem gleichen Labor) zu erfolgen. Anschliessend erfolgt keine Vergütung mehr.
Die Sanofi-Aventis (Schweiz) AG erstattet dem Krankenversicherer, bei dem die versicherte Person zum Zeitpunkt des Bezugs versichert war, auf dessen erste Aufforderung hin für jede bezogene Packung CABLIVI einen festgelegten Anteil des Fabrikabgabepreises zurück. Sie gibt dem Krankenversicherer die Höhe der Rückerstattung bekannt.
Erfolgt die Therapie bis zu einem ADAMTS13 Aktivitäts-Schwellenwert von mindestens 20% und nach Absetzen der Therapie tritt innerhalb eines Monates ein Rezidiv auf, erstattet Sanofi-Aventis (Schweiz) AG erstattet dem Krankenversicherer, bei dem die versicherte Person zum Zeitpunkt des Bezugs versichert war, auf dessen erste Aufforderung hin für jede bezogene Packung CABLIVI einen festgelegten höheren Anteil des Fabrikabgabepreises zurück. Sie gibt dem Krankenversicherer auch die Höhe dieser Rückerstattung bekannt.
Die Mehrwertsteuer kann nicht zusätzlich zu diesem Anteil des Fabrikabgabepreises zurückgefordert werden. Die Aufforderung zur Rückerstattung soll ab dem Zeitpunkt der Verabreichung erfolgen. Folgender Code ist an den Krankenversicherer zu übermitteln: 20955.01
