Vor Therapiebeginn muss für alle vergütungspflichtigen Indikationen eine Kostengutsprache des Krankenversicherers nach vorgängiger Konsultation des Vertrauensarztes eingeholt werden. Eine Kostengutsprache hat den entsprechenden Indikationscode (21692.XX) zu enthalten.
Die Diagnosestellung und die Verordnung von Concizumab, die Behandlungseinleitung und die darauffolgende Betreuung darf nur an folgenden universitären und/oder spezialisierten Hämophiliezentren erfolgen.
Dazu gehören: Kantonsspital Aarau, Universitätsspital Basel, Universitätskinderspital beider Basel, Ospedale Regionale di Bellinzona e Valli, Inselspital Bern, Kantonsspital Graubünden, Hôpitaux Universitaires Genève, Centre hospitalier universitaire vaudois, Centre Hospitalier du Valais Sion, Kantonsspital Luzern, Zentrum für Labormedizin St. Gallen, Ostschweizer Kinderspital St. Gallen, Universitätsspital Zürich, Universitäts-Kinderspital Zürich.
Die Kosten (inkl. Versandkosten) für den Concizumab-ELISA-Test werden von Novo Nordisk Pharma AG übernommen.
Die Preisberechnung für Blutpräparate erfolgt aufgrund des Fabrikabgabepreises zuzüglich einer Fixmarge wegen der speziellen Verteilersituation (praktisch kein Zwischenhandel) von Fr. 40.-- zuzüglich MWST.
Zur Prophylaxe von Blutungsepisoden bei Patienten ab 12 Jahren mit kongenitaler Hämophilie A und Faktor Vlll-Hemmkörper > 0,6 BU in der Anamnese, die die Anwendung eines Bypassing Agent-Präparates zur episodischen oder prophylaktischen Therapie erfordern.
Die Vergütung in Kombination mit der Anwendung mit Emicizumab ist ausgeschlossen.
