Die Behandlung bedarf der Kostengutsprache durch den Krankenversicherer nach vorgängiger Konsultation des Vertrauensarztes.
Die Roche Pharma (Schweiz) AG vergütet nach Aufforderung durch denjenigen Krankenversicherer, bei dem die versicherte Person zum Zeitpunkt des Bezugs versichert war, auf die Kombination von ZELBORAF und COTELLIC dem Krankenversicherer für jede bezogene COTELLIC Packung (=Indikator für einen Behandlungszyklus) 35.03% des Fabrikabgabepreises jeder COTELLIC Packung zurück. Die Mehrwertsteuer kann nicht zusätzlich zu diesem Anteil des Fabrikabgabepreises zurückgefordert werden.
Der Indikationscode dieser Limitierung ist an den Krankenversicherer zu übermitteln.
