Nach vorgängiger Kostengutsprache durch den Krankenversicherer nach vorheriger Konsultation des Vertrauensarztes. Eine Kostengutsprache hat den entsprechenden Indikationscode (20764.XX) zu enthalten.
Für VENCLYXTO wurde ein einheitlicher Abrechnungspreis festgelegt. Sind in spezifischen Fällen (z.B. Einzelfallvergütung) die wirtschaftlichen Preise in den jeweiligen Indikationen zu berücksichtigen, gibt die Zulassungsinhaberin auf Aufforderung der Krankenversicherung die Preise bekannt.
Als Monotherapie bei Erwachsenen zur Behandlung einer chronischen lymphatischen Leukämie (CLL), die eine 17p-Deletion oder TP53-Mutation aufweisen und die unter einer Behandlung mit einem Inhibitor des B-Zell-Rezeptor-Signalwegs ein Therapieversagen zeigten. Die Behandlung wird vergütet bis eine Krankheitsprogression oder inakzeptable Toxizität auftritt.

VENCLYXTO als Kombinationstherapie mit Rituximab bei Erwachsenen zur Behandlung einer chronisch lymphatischen Leukämie (CLL), die mindestens eine Vortherapie erhalten haben.
Die Behandlung (inkl. maximal 6 Zyklen Rituximab) wird für max. 24 Monate + 5 Wochen (Therapiedauer gemäss Fachinformation) vergütet.
Es werden keine weiteren anti-CLL Arzneimitteltherapien vergütet bis eine Krankheitsprogression auftritt.

VENCLYXTO wird in Kombination mit Azacitidin oder Decitabin zur Behandlung von erwachsenen Patienten mit neu diagnostizierter (therapienaiver) akuter myeloischer Leukämie (AML), für die eine intensive Chemotherapie nicht infrage kommt, vergütet. Ausgenommen sind Patienten mit akuter Promyelozytenleukämie (APL).
Die Behandlung wird vergütet bis eine Krankheitsprogression oder inakzeptable Toxizität auftritt.

VENCLYXTO als Kombinationstherapie mit Obinutuzumab wird vergütet zur Behandlung einer bisher unbehandelten chronisch lymphatischen Leukämie (CLL) bei Erwachsenen, die zusätzliche Komorbiditäten aufweisen:
• Kreatinin-Clearance • CIRS-Gesamtsocre (Cumulative Illness Rating Scale) > 6 oder einem vergleichbaren Wert eines anderen anerkannten Bewertungssystems.
Die Behandlung (inkl. maximal 6 Zyklen Obinutuzumab) wird für maximal 12 Zyklen vergütet.
