Nur nach vorgängiger Kostengutsprache durch den Krankenversicherer nach vorheriger Konsultation des Vertrauensarztes. Eine Kostengutsprache hat den entsprechenden Indikationscode (21290.XX) zu enthalten.
Die Behandlung wird bis zum Fortschreiten der Erkrankung (Progress) oder bis zum Auftreten unzumutbarer Toxizität vergütet. Klinisch stabile Patienten mit initialem Befund einer Progression können bis zur Bestätigung der Progression der Erkrankung weiter behandelt werden.

Als Monotherapie zur Behandlung von erwachsenen Patienten mit nicht resezierbarem lokal fortgeschrittenem oder metastasiertem triple-negativem Mammakarzinom, die mindestens zwei Vortherapien (Chemotherapie oder gezielte bzw. molekulare Krebstherapie (targeted bzw. moleculary targeted therapy)) gemäss gängigen klinischen Leitlinien erhalten haben, davon mindestens eine Chemotherapie (unabhängig vom Krankheitsstadium) sowie eine Vortherapie im metastasierten Stadium.

Als Monotherapie zur Behandlung von erwachsenen Patienten mit nicht resezierbarem lokal fortgeschrittenem oder metastasiertem Hormonrezeptor (HR)-positivem, humanem epidermalem Wachstumsfaktor-Rezeptor-2(HER2)-negativem Mammakarzinom, die eine Endokrin-basierte Therapie in Kombination mit einem CDK4/6-Inhibitor unabhängig vom Krankheitsstadium und mindestens zwei systemische Chemotherapielinien im metastasierten Stadium erhalten haben und die auf die letzte Therapie eine Krankheitsprogression aufzeigten.
