Die Einreichung eines Antrages um Kostengutsprache und die Erstverschreibung im Rahmen einer Kostengutsprache erfolgen ausschliesslich durch einen Facharzt für Hämatologie oder Onkologie, oder durch einen Facharzt für Dermatologie mit nachgewiesener Expertise in der Behandlung von kutanen T-Zell-Lymphomen, der an einer Weiterbildungsstätte der Kategorie A, B oder C mit Fachgebiet Dermatologie und Medizinische Onkologie oder Medizinische Onkologie oder Hämatologie tätig ist, die gleichzeitig mindestens einen dort tätigen Facharzt für Onkologie oder Hämatologie aufweist.
Die Kostengutsprache wird für eine Behandlungsdauer von maximal 1 Jahr gewährt und kann maximal jährlich erneut beantragt werden.
