Es gelten die entsprechenden Vergütungskriterien gemäss der Limitation für Pertuzumab (siehe Limitation PERJETA) und bezüglich der Rückerstattung für HERCEPTIN in dieser Kombination gilt zusätzlich folgende Limitation:
Die Zulassungsinhaberin erstattet dem Krankenversicherer, bei dem die versicherte Person zum Zeitpunkt des Bezugs versichert war, auf dessen erste Aufforderung hin für jede adjuvant (postoperativ) bezogene Packung HERCEPTIN in Kombination mit Pertuzumab (PERJETA) einen festgelegten Anteil des Fabrikabgabepreises zurück. Sie gibt dem Krankenversicherer die Höhe der Rückerstattung bekannt. Die Mehrwertsteuer kann nicht zusätzlich zu diesem Anteil des Fabrikabgabepreises zurückgefordert werden. Die Aufforderung zur Rückerstattung soll ab dem Zeitpunkt der Verabreichung erfolgen.
Folgender Code ist an den Krankenversicherer zu übermitteln: 17354.01
Für die zugelassenen Indikationen und für andere Kombinationen, sofern entsprechend Limitierung der Kombinationspartner eine Vergütung vorgesehen, gelten die in der SL aufgeführten Preise (ohne Preismodell) von HERCEPTIN.
