Kriterien für die Kostengutsprache:
1. Metastasierende Basalzellkarzinome.
2. Lokal fortgeschrittene Basalzellkarzinome, bei denen chirurgische Massnahmen oder Strahlentherapie nicht angemessen sind:
2.a. Zustand nach operativer Entfernung von mehreren (> 5) Basalzellkarzinomen, wenn sie im Rahmen eines genetischen Syndroms auftreten, wie z.B. dem Basalzellnävussyndrom (Gorlin-Goltz-Syndrom) oder im Rahmen von Xeroderma pigmentosum.
2.b. Ein Basalzellkarzinom mit einem Durchmesser von mind. 10 mm oder ein Basalzellkarzinom, das nach zwei therapeutischen Eingriffen mit kurativer Intention (Radiotherapie und/oder operative Therapie) rezidiviert ist oder in einem Körperbereich lokalisiert ist, wo ein Eingriff mit kurativer Intention (Radiotherapie und/oder operative Therapie) zu bleibenden, entstellenden Veränderungen führen würde. Problematische Lokalisationen in diesem Zusammenhang können z.B. der zentrale Gesichtsbereich oder die Ohren aber auch genitale, perianale oder gelenknahe Prozesse (z. B. axillär) sein.
2.c. Basalzellkarzinome, die bei der Diagnose weit fortgeschritten sind, bei denen mittels bildgebender Verfahren oder histologisch eine Invasion in benachbarten Strukturen wie Knochen, Nerven, Muskulatur dokumentiert ist und eine Therapiemassnahme mit kurativer Intention (Radiotherapie und/oder operative Therapie) zweifelhaft ist.
2.d. Basalzellkarzinome, die sowohl operativ und radiotherapeutisch vorbehandelt sind und wo aufgrund der Vorbehandlung die Tumorausdehnung klinisch auch unter Einsatz bildgebender Verfahren schwer abgrenzbar ist.
2.e. Basalzellkarzinome bei Patienten, bei welchen Strahlentherapie kontraindiziert ist.
3. Fortgeschrittene Basalzellkarzinome bei Patienten, bei welchen aufgrund einer internistischen Grunderkrankung eine Operation oder Strahlentherapie nicht zu verantworten wäre.
