Die Therapie bedarf der Kostengutsprache durch den Krankenversicherer nach vorgängiger Konsultation des Vertrauensarztes.
Als Monotherapie oder als Kombinationstherapie mit topischen Kortikoiden zur Behandlung erwachsener Patienten (ab dem 18. Lebensjahr) mit schwerer atopischer Dermatitis (IGA 4 auf IGA Skala von 0-4 oder SCORAD > 50 oder EASI ≥ 21.1 ), sofern die Patienten auf eine intensivierte Lokalbehandlung mit verschreibungspflichtigen topischen Therapien (topische Kortikoide und/oder Calcineurininhibitoren) und Phototherapie (sofern verfügbar und angezeigt) und auf eine systemische Behandlung mit einem konventionellen Immunsuppressivum (ausgenommen systemische Kortikoide) während mindestens einem Monat unzureichend angesprochen haben oder bei denen diese Therapien kontraindiziert sind oder aufgrund von klinisch relevanten Nebenwirkungen abgebrochen werden mussten.
Abrocitinib wird nicht in Kombination mit anderen systemischen Arzneimitteln zur Behandlung der atopischen Dermatitis vergütet.
Falls nach 12 Wochen Behandlung mit Abrocitinib kein therapeutischer Erfolg eingetreten ist, d.h. keine IGA Reduktion um ≥ 2 Punkte gegenüber dem Ausgangswert oder keine ≥50% Verbesserung des EASI-Scores (EASI 50) gegenüber dem Ausgangswert oder keine ≥50% Verbesserung des SCORAD-Score (SCORAD 50) gegenüber dem Ausgangswert, ist die Behandlung abzubrechen.
Nach 52 Wochen ununterbrochener Therapie der atopischen Dermatitis mit Abrocitinib ist eine erneute Kostengutsprache durch den Krankenversicherer nach vorgängiger Konsultation des Vertrauensarztes einzuholen.
Die Diagnosestellung, die Verordnung und die Verlaufskontrolle von Abrocitinib in der Indikation atopische Dermatitis darf ausschliesslich durch einen Facharzt für Dermatologie und Venerologie oder Facharzt für Allergologie und klinische Immunologie erfolgen.
